Low Back Pain

Up-to-date practice guidelines from the American College of Physicians recommend non-pharmacologic therapies as the first line of treatment for acute, subacute, and chronic lower back pain. This includes exercise, spinal manipulation, and other non-pharmacologic interventions. For chronic lower back pain, the guidelines add cognitive behavioral therapy and mindfulness-based stress reduction to the list of appropriate treatment options.


The guidelines do not break down the treatment options for the different types of back pain. There are more advanced algorithms and guidelines updated in 2021 using treatment-based classifications categorize patients based on their clinical presentation. From there, we can select a more specific treatment option and can develop a personalized treatment plans that address the specific needs of each patient.

There are several treatment-based classifications for lower back pain, including manipulation, specific exercise, stabilization, traction, and directional preference. Manipulation involves the use of high-velocity, low-amplitude thrusts to restore joint mobility and alleviate pain. Specific exercise involves targeted exercises to address movement restrictions and muscle imbalances. Stabilization involves exercises that target the deep abdominal and back muscles to improve stability and prevent further injury. Traction involves the use of mechanical forces to decompress the spine and alleviate pain. Directional preference involves identifying the specific movements or positions that exacerbate or alleviate symptoms, and developing an exercise program based on these preferences.
